I hereby agree to the collection, retention, and processing of personal data contained in this form by the administrator, MAFIA RECORDS s.r.o., registered office at U Průhonu 1588/11a, 170 00 Prague 7, Czech Republic, Ident. No. 28103319 (and/or its staff), for the purpose as defined below. This consent has been granted for all data contained in this form, for the period of 5 (five) years after the date of the consent. I am aware of my rights pursuant to sections 12 and 21 of the Act No. 101/2000 Coll. on the Protection of Personal Data. I have been familiarized with all filled-in sections of this form and I declare all data are exact and true and have been provided voluntarily.

Purpose of processing of data

Pursuant to Section 5 of the Act No. 101/2000 Coll. on the Protection of Personal Data, all data provided in this form are collected and processed solely for the purpose of sending commercial messages to the data subject using electronic means as defined in the Act No. 480/2004 Coll. on Certain Information Society Services, until the data subject has sent the information (directly and effectively) that he/she does not want to receive commercial information from the administrator any longer. However, this will apply for a maximum of 5 (five) years. The administrator can use summarized data contained in this form for statistical purposes or mainly for the internal needs of the administrator.

Statement of the administrator

The administrator represents and warrants all personal data will be collected to the extent as necessary for achieving the specified purpose and processed solely in compliance with the purpose they have been collected for. The employees of the administrator or any other natural persons processing personal data based on the agreement with the administrator, as well as any other persons, are obliged not to disclose personal data, and this obligation shall survive after the termination of their employment or works.
